Progress notes are among the most important reports that a medical transcription company helps physicians document in the electronic health record. These brief notes, also called SOAP (Subjective, Objective, Assessment, Plan) notes, document various aspects of the patient’s treatment and highlight important issues or concerns relating to care.

Purpose of Mental Health Progress Notes

  • Subjective – The patient’s description of the problem/intervention
  • Objective – The therapist’s objective observations
  • Assessment – The therapist’s analysis of the various components in the subjective and objective sections of the note
  • Plan – A description of the treatment plan to the reach the goals or objectives (including additional tests to rule out or confirm a diagnosis, prescribed medication or surgical interventions, and follow-up).

I think that doing a good treatment plan makes progress notes easier to write as well as keeping the therapeutic enterprise focused.

Meaningful progress notes meant I could review them prior to a session and have a good idea of where the client and I had been, where we were and, the options and directions we had for the upcoming work.

The real strategy for keeping up with notes was to never leave work until they were done. If it meant having a working lunch or staying late, so be it. Once the camel’s nose was in the tent, you never caught up.

For a while my agency experimented with dictating progress notes, evil’s and treatment summaries. Heaven on earth until the bill came in.

The volume of paperwork for substance abuse was, frankly, depressing. You didn’t keep up. You just kept adding to the pile and hoped for a snow day.

Once a year the state would send out a team of examiners to make sure there was no fraud (billing for phantom sessions), the notes reflected with clarity and clinical astuteness the work being done and, lived up to the level of quality demanded by the State of Maine regulations governing mental health and substance abuse agencies. Charts had to be reviewed by the clinician as well as be peer reviewed by co-workers. Nights and weekends for this one.

What are the ideas to be kept in mind before writing a note?

Copy: If you’ve scribbled down notes in a hurry, you might not be able to read them when it comes time to prepare for a test.

Compare: Keep your notes handy when you’re doing your reading assignments. Compare what you wrote with what the readings say — you can add to your notes as you read.

Write down key factors: If you have a teacher who writes notes on the board, that’s a bonus: You can copy them down.
